I understand that I will be provided the following: 1-Football Helmet 1-Pair Shoulder Pads 1-Game Jersey 1-Practice Jersey 1-Game Pants By digitally signing\agreeing, I am accepting responsibility to return the above equipment in good condition and when requested at the end of the season. If I do not return the equipment, I am authorizing Waverly/Eagle Jr Vikings to cash/keep my equipment deposit of $275 to replace the above items.

Jr Vikings Football - Refund Policy *

By digitally signing\agreeing, I understand that I am accepting Waverly/Eagle Jr. Vikings Youth Football’s refund policy as outlined below: -Until practice starts, you are entitled to a full refund, minus $100 for admin/league fees. -If your child quits, is pulled from the team, or incurs injury (eliminating he\she from continuing on the year) after the start of practice and before the 1st league game, you will are eligible for 50% of your fees refunded to you. -If your child participates in the 1st game, you will forfeit all fees paid to date, and no monies will be refunded.
